Licensed Practical Nurse (Part Time Position)

GENERAL DESCRIPTION:

Under the supervision of the Clinical Director and the Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides oversight of the nursing needs of individuals, coordinates the RN delegation training to staff, participates in the development and maintenance of the residential care and treatment program according to the medical needs of each individual person, and provides on-call assistance.

ORGANIZATIONAL RELATIONSHIP:

The LPN is responsible for directing the medical activities of the Medical Assistant (MA), and provides relevant information described below to the Clinical Director, RN, and/or Executive Director.

QUALIFICATIONS:

Licensed Practical Nurse with the State of Tennessee Department of Health
Minimum of 2 years’ experience in nursing
Experience in the treatment of individuals with developmental disabilities
Two years of case management experience preferred
Excellent verbal and written skills
Excellent interpersonal skills, with ability to observe, question, listen actively, give feedback effectively in a consultative team approach, and deal effectively with conflict, all while maintaining objectivity, diplomacy, and tact
Keyboarding skills and computer literacy for purposes of using word processing applications, reports, spreadsheets, and charting
Excellent problem-solving and decision-making skills
Ability to work independently with strong skills in time-management, multi-tasking, delegation, and follow-up skills, with the ability to plan, organize, and prioritize a large workload
Flexibility in assigned working hours
Possess a high energy level, adaptability, assertiveness, patience, and ability to work in a team environment
Driver’s License
Must be able to lift and transfer 50 pounds

DUTIES AND RESPONSIBILITIES:

Assist in maintaining a quality of care that is representative of WTFS standards
Assist in the completion and oversight of accurate documentation of home visits and medical documentation for purposes of compliance with state and agency standards
Assist in delivery of medication to individual homes
Instruct staff on medication, dosage, administration, side-effects, and any other pertinent information
Monitor the Medication Administration Records and notify responsible parties of any errors
Assist in individual health assessments, and contact relevant staff for follow-up on injury, illnesses, etc.
Train staff on plans of care, individual specific health issues, medications, and other health-related items
Coordinate and schedule medical appointments, and attend appointments with individuals and staff as needed
Notify staff of any specific details related to medical appointments and needs (e.g. NPO, fluid restrictions, etc.)
Review medical orders and recommendations and ensure that the information is communicated to responsible parties
Collaborate with members of the individuals’ planning team to provide medically relevant information
Maintain proper documentation according to policy
Maintain all nursing licensure responsibilities
Participate in weekly staff meetings and Incident Review Committee
Participate in the Administrator on Duty on-call rotation system
Any other duties as assigned by the Clinical Director

Teaching Counselor
Full or Part time/Non-Exempt

Job Summary:

The primary responsibility of the Teaching Counselor is to teach skills and behaviors to maximize independence in activities of daily living for clients who have developmental disabilities and other special needs.

Qualifications:
High School diploma or equivalency required.

One – Two (1-2) years of prior experience in a position of similar scope, preferred.

Ability to lift at least one-fourth of own body weight unassisted and ability to perform unassisted transfers of disabled adults with adaptive equipment (lift belts, Hoyer lifts, etc.).

Knowledge of Teaching Family Model components and techniques preferred.

The ability to handle all aspects of personal care with sensitivity and caring.

Strong verbal and written communication skills.

Operation of a motor vehicle is required which includes proof of a valid driver’s license and an ability to qualify as a driver under WTFS insurance guidelines.

Completion of WTFS and DIDS training curriculum within specified time frame.
